2) File a complaint online or see the Department Contacts to call, fax or e-mail your complaint. You can also mail this information, but remember, the sooner we receive your complaint the better.

 

Please note that complaints can be called in on an anonymous basis, but if you want to know the results of the inspection, we will need your name and phone number.
